I eat food with Dave's Ghost Pepper Naga Jolokia Sauce without any affect. After putting Mad Dog 357 on my food I'm like good God, I can actually burn again! It's glorious. Since I just got it I'm not sure how long it may take to become acclimated, but I am happy. The taste is great and it adds fantastic heat. Anything that adds something remotely chili flavor and not some citrus-nonsense it a plus in my book.
The flavor isn't exactly chili, but it's in the ballpark. Not exactly sure what it is, except delicious. It's hot and wonderful. Well worth the purchase.
Finally, I decided to mix up my own sauce. It's nothing sophisticated: some tomato-based Ortega, a generous portion of Marie Sharp's delicious Habanero sauce, a splash of Truff Hotter Black Truffle Hot Sauce, and a few drops of Mad Dog. The results were magical. Smoothness, rich flavor, and heat. So, so much heat.
As far as I'm concerned, this stuff is about as edible as napalm straight out of the bottle. However, if you get creative with how you use it, it's an absolutely brilliant ingredient.

I think if one were very careful with adding drops, it could make an amazing and unique spicy mayo with lots of uses.
One note I haven't seen mentioned in the reviews yet- you can taste the cane extract, and it makes it sweeter than you'd expect. When I first tasted it, I thought it was cinnamon or cloves. So depending on how you want the predominant flavors to taste, this might not be for you. Despite the extreme heat, this sweetness comes through pretty strong. I prefer a more sour, vinegary, or limey flavor so just keep that in mind, YMMV.

I had never really used anything hotter than Tabasco untill the beginning of this quest, so I started with a bottle of Tabasco Habanero sauce, when I was close to finishing this I ordered a bottle of Mary Sharps Comatose sauce, which was pretty intense for me! Maybe 3 quarters through that bottle I decided I would go with Tabasco Scorpion sauce next. It was pretty darn hot! But honestly not the mind-blowing over the top heat you would expect given the reputation of the pepper and the advertisements on the bottle.
So after much deliberation I decided to go with the fairly notorious Mad Dog 357 original hot sauce as my next step up the hot sauce ladder. I read a lot of reviews saying that Dave's hot sauces were not as spicy as advertised and that Da Bomb simply tasted awful...
2 drops on a piece of pizza the first day it arrived and I was in a pool of sweat, stunned with my sinuses dripping all over the place. This is very much the first hot sauce I have had that gives me a real feeling of nervousness before I take it off the shelf, it is not "spicy", it is painful!
The flavour is deep and smoky, not at all like Tabasco, not terribly strong, the heat definitely comes first..
This will take me a while to finish.

Bought this along with a couple of other ridiculously hot sauces. This is actually relatively flavourful compared to some of the others, but does still have that hot sauce tang. Beware, this will blow your head off. It hurts. We do actually use this in cooking, though, just adding a drop to curries or pasta sauce, etc, it really does add a nice heat. (When I say a drop, I mean a drop).
